2025 Plan Changes
NEW! Hinge Health Musculoskeletal Pain Support : In partnership with Anthem, we are excited to offer this musculoskeletal health resource at no cost to Anthem members and their dependents. Hinge Health provides convenient, effective, virtual physical therapy sessions you can do anywhere to help reduce pain, recover from injury, and move better.
Hinge Health offers:
- 1:1 support from a physical therapist or health coach
- A personalized care plan designed to fit your everyday life
- Convenient sessions with the Hinge Health app

NEW! Modern Health enhancements: We’re happy to include new enhancements to our Modern Health benefit. Modern Health provides you and your dependents with first-class resources that support your emotional, physical, and even financial wellness—all in a single, secure platform.
Enhanced care for 2025 includes:
- 10 coaching and 10 therapy sessions
- Unlimited text with therapists and coaches
- Unlimited live and pre-recorded group sessions
- Self-guided, in-app programs and courses on important mental health topics

Pharmacy Benefit Changes: CVS 90 Network will now be your exclusive 90-day retail prescription provider.
- Fill your 90-day maintenance prescriptions at CVS retail locations (9,900 nationally), or use Optum’s mail-order pharmacy, for cost savings and enhanced service.
- 90-day prescriptions filled through CVS or Optum mail-order pharmacies will be discounted to 2.5x the 30-day retail copay. If you fill a 90-day prescription at a non-CVS or Optum mail-order pharmacy, your copay will be 3x the 30-day retail copay.
HSA/FSA Pre-Tax contribution limits:
- The annual IRS contribution limit for Health Savings Accounts (HSAs) is increasing to $4,300/$8,550 (individual/family). If you are age 55+ you can contribute an additional $1,000 per year to your HSA.
- The contribution limit for the Healthcare Flexible Spending Account (FSA) is increasing to $3,300. You may elect up to this limit and will be contacted once the 2025 limit is released if it differs from the amount in the system.

Other Important Considerations
- Flexible Spending Accounts (FSAs): If you want to participate in the Health Care FSA, Limited Purpose FSA, Dependent Care FSA, and/or Commuter FSA, you must enroll (or re-enroll) during Open Enrollment—your previous elections do not carry over!
- Health Savings Account (HSAs): For those enrolled in the HDHP HSA 2500 medical plan, your HSA elections do not roll over to the next plan year. Please re-elect HSA contributions for 2025 during open enrollment. You can change your HSA contributions anytime throughout the year.
- No-Cost In-Network Preventive Care: As a reminder, in-network preventive care is fully covered under each medical plan.
- Paycheck Contributions: Radiology Partners is still paying for the majority of the total plan costs.
- Dependent Verification: Dependent verification will be required when enrolling a dependent in coverage. See the Enrolling & Eligibility page for additional details including acceptable documentation.
Remember: The choices you make during Open Enrollment will remain in effect for the entire 2025 plan year, and you will not have another chance to make changes until the next Open Enrollment (unless you experience a Qualifying Life Event).

Making Changes to Your Benefits
Once you choose your benefit options, you cannot make changes or additions until the next annual Open Enrollment unless you have a qualifying life event. Teammates have 30 days from the date of a qualifying life event to add, change, or terminate dependent coverage during a non-open enrollment period.
If you do not add, change, or terminate elections during the 30 days, you will have to wait until the next plan year’s open enrollment or experience another qualifying life event.
Qualifying Life Events include:
- Marriage
- Divorce or annulment
- Dissolution of domestic partnership
- Legal separation
- Birth of a dependent
- Adoption/placement for adoption
- Gain of other coverage
- Loss of other coverage
STEP 1: LOG INTO UKG
Once you log in to OKTA, go to the dashboard and select UltiPro.
